Is this in the open air, without any obstructions above and around the saplings?
I'm thinking that if the game (randomly) chose a tree variant that is too tall for the available space, it might fall back to growing a tree from one sapling alone.

It always happens when growing a large tree under a large jungle tree branch, I think you found the culprit.
